I have the same problem and the boxes are different. For me it asks the distribution code in box 7 which is 7. Then 3 questions later it asks where the distribution came from and 04 is one option( only option that pertains to me). But I just put in 7 when It asked the code in box 7. Lol. So i tried "none of the above and I tried 04. The state return remains the same.
