Also, a 401(k) is not an IRA, so if the distribution was from a 401(k), only the exceptions that are available for distributions from a qualified retirement plan would apply, not those that only apply to IRAs. The page shown by MindyB is for distributions from an IRA. The similar page for distributions from a 401(k) has a slightly different list.
