without the "return excess with earnings" form. you did on your own a simple distribution which can be an early distribution depending on your age. do you still have the 1099-R for that? it may be online.

I am wrong about "all the other parts" but my question was about having done a distribution from a tIRA and then within 60 days having made contributions to a ROTH IRA. Without telling the institutions it was reported on the 5498 as a contribution.
I have gone through and amended and/or created missing reporting form 5329 and 8606 for all years. My lesson is about involving the institutions to get the reporting done correctly, and leaving enough time before 4/15 to take care of any of these details.
