TurboTax will not put the distribution on lines 5a/5b if the IRA/SEP/SIMPLE box is marked on TurboTax's 1099-R form. Make sure that you have marked that box in agreement with the marking on the from provided by the payer. It seems that you have not marked this box when entering your spouse's form.
