Retirement tax questions

I have two W2s as I switched employers last year. Both W2s seem to be entered correctly with no pension codes used in box 12. Boxes 12, 13, & 15-17 are not empty, but this is necessary to document my contributions and state tax/tax withholding information. 

The M1QPEN form is automatically entering the name of a financial institution that provided me with a 1099-R that I entered in the federal retirement section of the income page. I already double-checked all the entries and they match the information on the 1099-R that I was provided. Are there any particular entries I should triple-check on this form? I would note that they did use the correct distribution code (H) in box 7 and boxes 14-19 are left blank on both the 1099-R and on TurboTax.com.
