Remember, you are subject to 4 types of tax withholding. Federal income tax, state income tax, social security tax, and medicare tax. Depending on your income and W-4, you might see no federal tax withholding even though you have state or social security withheld. That could be normal.
However, if you clearly have federal income tax withheld from your paycheck, that is supposed to be added up and put in box 2, and that has nothing to do with how you fill out your W-4. It's a plain mistake by your employer or their payroll processor.
