Thank you for your detailed answer. That did the trick. The next screen after the one you posted also asks: "Was this distribution from a Qualified Plan?" "Commonwealth of Massachusetts" I'd selected the "Qualified Plan" button (vs. "Non qualified plan") on that screen assuming it would take care of it. Your screen shot was worth its weight in gold.
