Trying answer your questions:  These screens are produced when entering RRB-1099-R, but you mentioned RRB-1099.  There are two different forms. RRB-1099 is the Blue form and RRB 1099-R is the Green Form.  I am talking about the Green form.  The questions come up after entering box 3, 4, 6, and 9.
