I found that the the DOS 1099-R follows the format found in the "CSA 1099-R" form, so within Turbo Tax, first choose the "CSA 1099R" option when choosing the type of form you received. I also found that dental and medical can be input under "Deductions & Credits" - "Medical". Hope this helps.
