The outer portion of the "envelope" I received contains the phrase, "2022 CSA FORM 1099R," then directly below that, in red, it states, "IMPORTANT TAX DOCUMENT ENCLOSED."
However, on the actually "[s]tatement of annuity paid" it states, "Form: 1099-R."
