Your employer gets the deduction for the employer contribution. You do not. The employer contribution does not get reported anywhere on your tax return.
As for your employee contributions, enter amounts shown in box 12 of your W-2 only in box 12 of TurboTax's W-2 form, nowhere else in TurboTax.
