Yes, you do need to report your RMD on your tax return.  The account owner is taxed at his/her income tax rate on the amount of the withdrawn RMD but if the RMD is a qualified distribution from a Roth IRA, it is tax free. Also, any part that was already taxed is tax free (to the extent the RMD is a return of basis).   You should have received a 1099R for your distribution from your bank.  You should contact your bank again to request the 1099.  If you do not receive it, please review IRS topic What to do if I do not receive my W2 or 1099R?
