Thank you so much for the reply. I did originally ask my new custodian to initiate these but all three holding institutions refused to work directly with them. Their lack of customer care was just one of many reasons I wanted to leave all of them. I also pushed as hard as I could to have the 1099-R from Y revoked, but they were adamant in their stance, stating their Legal Team made the call. Of course you can't get through to the "Legal Team" unless you hire an attorney to do so.
I only have one more IRA to move so I won't ever have this dilemma again, but your answer makes me feel much better about this situation. Thanks again, and Happy Holidays!
