If any state tax was withheld by the Pension or IRA administrator, then entries in 14, 15, and 16 are required by the state, not just TurboTax.
However, as DoninGA said above, if there is no state tax withheld, there you do not need any entries in these three boxes.
The problems occurs when a fund administrator makes an unnecessary entry in one of the boxes when there was no state tax withheld. In this case, TurboTax has to ask you if this is correct, because we would not want you to accidentally leave out an important field.
