Did you actually have a 401K distribution, or is this where you are directed to for an error?

Start at Wages & Income
Scroll down and select IRA, 401(k), Pension Plan Withdrawals (1099-R)
This may say Review
- Have you ever taken a disaster distribution before 2021? = Yes
- Did either of you take a disaster distribution at any time between 2018 and 2022? = No
