We received unemployment benefits (paid state tax) and the only form we get is a 1099-G statement (Copy B). Do we need to attach this to our IL-1040?

We received unemployment benefits (paid state tax) and the only form we get is a 1099-G statement (Copy B). Do we need to attach this to our IL-1040?
Copy B is for us. Copy A was sent to the IRS by IDES. There is no other form (i.e. Copy 2 for local or state tax filing).
