There is a form on which this will be reported, Form 8915-F. (Last year you used Form 8915-E). You will not receive a 1099-R for 2021 for your election, because that entire income was reported in 2020 already. The election from Form 8915-E carries over in the IRS database, and you will be expected to file the new form this year, which has yet to be released by the IRS.
