I assume that this is a box 7 Code G.
A 401(k) code G rollover is automatically a rollover to a Traditional IRA *unless* it was converted to either a 401(k) Roth or a Roth IRA. If neither of those then then it *is* a rollover to a Traditional IRA. Just answer "no" to the two Roth questions and you are done - there is no other choice.
