Retirement tax questions

Thank you for the reply. But this does not answer my question. 

 

I have listed lump sum amounts for each year, ie: 2019, 2018, 2017, & 2016. Then it has listed one more line "paid in 2020 for other tax years". This is where my issue is, as the program asks for a specific year and this last line is for 2015 & 2014 as my SSDI benifits started in 2014. So I have no way to enter those last lump sums as the form has them together without a specific year.
