Thank you SO much for taking the time to go through this very detailed exercise, especially since the IRS instructions failed to fully address the situation. The custodian was VOYA, so not a small bank. They failed on this issue, and also issued a second 1099-R for a recharacterization using the wrong year code so we're waiting on that correction as well.
