Thanks for the screen shots!
The difference between your entries and mine are that I selected the response that said "Joe moved the money to another retirement account (or returned it to the same retirement account)."
After I saw your screen shots, I confirmed that if I select "Joe did something else with it (cashed out, etc.)", then box 4a is left blank.
