Retirement tax questions

Hi, I do have the box checked. It doesn't go to any other link to ask anymore questions related to COVID. 

It goes to asking if I rolled over the money (which I didn't), then it goes to asking about the special averaging method and tells me I do not qualify. Then asks if I ever took a disaster distribution before 2020, which I have not. Then it takes me to this screen about not paying the additional tax-- nothing is related to COVID relief. The next screen says I have to pay the 10% penalty. Which I know for sure that I should not, it is a qualified retirement plan and only took out $10K which was directly related to being laid off of work due to quarantine from COVID.
