1099-R input issues, do I need 4852 if correct and how do I get one?
I had a 403b Roth and rolled it over to a Roth IRA. I received the 1099-R form, but it doesn't have the "box 9," so I had to check the "Substitute Form 1099-R" and enter the information that way, even though the form is not in error. Now when I try to file, it tells me I need a "Form 4852: Substitute Form W-2, 1099-R." I'm assuming that part of this is that my work goofed (not surprised) on the W-2 and I need a new one, but in addition, I need a 4852 (?) and additional 1099-R? I spoke to Nationwide and they confirmed that the information on my 1099-R is accurate, and so are the codes in box 7 (listed as BG, but BH would also be accurate). It looks like nobody (?) is allowed to e-file this year from other comments I'm seeing, but am I only waiting on the replacement W-2, or is there some other form I am supposed to be waiting on?
