I cannot reproduce that.   I suggest deleting all 3 1099-R's.    Change your DOB first if it falls into the range of not asking the question and re-enter the 1099-R's.

 

Be sure to answer the RMD question that it was NOT required for all 3, since there was no 2020 RMD (but that should not affect the QCD.)

 

When I enter 3 1099-R's 2 for the spouse (one inherited) and 1 for the taxpayer the QCD amount is correct.
