You do NOT use a code G unless a code G was on the 1099-R that you received.

You do NOT use a substitute 1099-R (4852) to do this.  Filing a 4862 is sure to generate an audit letter from the IRS.

 

What code is in box 7 on the original 1099-R?

Is the IRA/SEP/SIMPLE box checked?
