I'm having the same issue (my employer didn't include pre-tax contributions to my HSA on my W2 in box 12), but I haven't seen the dialogue "Did your employer tell you about any other contributions? Employer and payroll contributions not reported in Box 12 of your W-2."
How do I get to this dialogue?
