I ran through the math which is in comments below. I did it quickly so please check. Compaq share exchange http://h30261.www3.hp.com/faq/faq-about-compaq-share-exchange.aspx  just allocate all to HP. 
In 2015 Split of HPQ and HPE  47.15% basis to HPQ and 52.85% basis to HPE http://h30261.www3.hp.com/faq/separation-faqs.aspx For your US basis per the forms above I calculated: 2015 split you have now 759 share of HPQ  with 47.15% of the basis or $27,593.24 or 36.35 a share;  and 759 shares of HPE with 52.85% of the basis or $30,929.01 or $40.75 a share.

 It is all long term so just report date purchased as various, no need to report lots.
