I was not seeing the "Qualified Dividends and Capital Gains Tax Worksheet" on my pdf copy of my return. I went back into Turbo Tax and hit amend return (i did not actually amend the return, I hope), then I wet to print return and it gave me the option to include all the forms. Once I did this and saved the pdf I was then able to see the "Qualified Dividends and Capital Gains Tax Worksheet".
