If you are getting questions/statements on those aspects you mentioned
- Capital Gain or Loss from Assets Acquired After December 31, 2011
- Gain, long term,
that's due to some other document (e.g. 1099-B) not a 1099-DIV.

make sure you entered all the payer information. TurboTax is fussy that way.
