Your description of a problem doesn't match the form.
I'm not familiar with WI. That said, if you think the form needs correcting, I suggest correcting it and sending it to the Wisconsin Dept. of Revenue (WDOR), with a cover letter explanation and copy of the original form.
The purpose of the form appears to be to track carry over contributions so that you can deduct, in future years, what you contributed but were not allowed to deduct this year. It sounds like something WDOR will be checking on future tax returns.
