Since New Hampshire does not have a state income tax, you will need to download and send the form directly to the NH Dept of Revenue to receive the credit.
Here is a link to download the form. Mail your completed Education Tax Credit Application (Form ED-02) to the New Hampshire Department of Revenue Administration at: NHDRA Education Tax Credit P.O. Box 637 Concord, NH 03302-0637. There is also an instruction on page two of this form on how to file online.
