On the 1040 it has printed beside line 1 "sch and an amount".
That is the correct way to report it. Note that the "sch amount" has been added to your W-2 wages amount and is included in the total amount shown on line 1 of form 1040. It does not get shown anywhere else on the forms or schedules.
