There is no IRS form. The calculations will only show on a TurboTax (TT) worksheet. TT will do that automatically; you don't initiate it.
The lack of an IRS form is a problem. We see frequent questions, here in this forum, that taxpayers have gotten notices from the IRS because they didn't report the full 1099-Q, box 2 amount, as income. The TT worksheet and proof that you paid expenses (bills & statements from then school) will take care of that.
