The federal employer identification number (EIN) issued for the LLC is actually associated with the business name (the first four letters). You can leave the business name if you like.
If you prefer to use your the personal name of your spouse, you can use 'Personal Name-LLC Name' all in the same field slot so that your spouses' name shows the way you wish. Either way would work for filing your 1096 and 1099s.
