TurboTax for California State Tax Returns includes Form 568. If this form should be associated with a Schedule C, go to Forms mode and check the box on the California Schedule C Worksheet for "Single member LLC".  Form 568 will then be generated, pulling information from your Schedule C entries.

 

If your LLC is associated with a rental property, look for a similar box on the California Schedule E Worksheet like the one for Schedule C. Again, Form 568 will be created.

 

Note that TurboTax will create and e-file only ONE Form 568 per California return. If you have more than one, you will need request an e-file waiver and mail your entire State return. If you look on Form 568, p 1-2, at the bottom, there is a notice. If the box is checked, you have to mail the form, as it can't be e-filed. But if that box is not checked, it appears the form can be e-filed with the rest of your return. As long as your Form 568 does not include one of those exceptions, it can be e-filed.
