The information contained within the form is generally not entered on your tax return unless you then sold the stock, or if you could be subject to alternative minimum tax (AMT). From you question, it appears the company sold your fractional shares; however, you also mentioned that you have never received the proceeds.
At this point, what are you being asked to verify? Are you being asked to verify that all of the information on Form 3921 (which you haven't received) is accurate? It seems you are not in a position to verify anything on Form 3921, because you don't have it. In other words, you don't know what it contains. Given that you don't have Form 3921, are you reporting sale proceeds from the sale of the fractional shares? If so, and you are confident that the entries are correct, can you confirm with your former company that the information you have entered is correct? If you can, and that information is the same that would appear on Form 3921, then you should be able to verify Form 3921, even though you don't have it, because you were able to verify the Form 3921 information independent of having the form.
