If Medicare part D recipient is the SPOUSE of the self employed person, where does that part D amount go? SSA form or schedule C?

Instructions for Medicare part D and B fees say:  "Note: If you are self-employed, you may enter the Medicare D premiums as Self-Employed Health Insurance on the business activity form (Schedule C, F, etc). Do not report Part D premiums on this worksheet and on other business activity forms."

However, if the spouse is the one who is self employed, does the spouse add in the Medicare fees to her schedule C?  The above instructions don't define who "you" is.
