Did you receive any regular wages beside the health reimbursements?
Your W-2 is wrong, so that needs to be corrected. Box 1 needs to be the total of (1) your regular wages, plus (2) your reimbursed medical premiums, plus (3) your reimbursed HSA contributions. Boxes 3 and 5 should only be based on the 'regular' wages.
