I have the same version as you, all I did is click on OPEN FORM, enter 3115 in the search box, and it found it. I think double click to open a copy. When you have found the 3rd Quick Link near the top and used it to create your statement, the statement becomes yet another form in your list. I deleted the 3115 itself later, it relates to change of accounting period and I didn't want or need it, it was blank anyway. I suspect if I went on to filing near the end when it checks errors and cleans up it eliminates totally blank forms you have opened which are unnecessary, and would have purged 3115 anyway. But the statement survives as a separate page.
