I am assuming that you are a Wisconsin resident, who is seeing the screen shot "Adjustments for pre-2011 contributions"?
If you made contributions to an HSA prior to 2011, Wisconsin did not permit the deduction of such contributions. However, subsequent to 2010, Wisconsin passed a law that enabled you to make adjustments to your current income in order to simulate taking the HSA contributions prior to 2010.
NOTE: this applies ONLY if you were in Wisconsin prior to 2011, filing a state return, and contributing to an HSA at the federal level. If not, leave the section blank and continue.
If you have no idea if you still had pre-2011 dollars in your HSA in 2017, then just leave this screen blank and continue.
This screen is not required in order to file your Wisconsin return.
You need to complete this screen only if you want to deduct dollars contributed to your HSA prior to 2011 while living in Wisconsin that still by some miracle happen to be in your HSA in 2017, AND which you spent on qualified medical expenses in 2017. If even one of these things does not apply, just leave the screen blank and continue.
