Did you make a contribution to your HSA through payroll deduction? Check box 14; sometimes employers mistakenly place the contribution there.
If that is the case, ask your employer why they did not use box 12 with a code of W as the IRS W-2 instructions say. Also ask them if they removed the amount of the contribution from Wages in boxes 1, 3, and 5 on your W-2. (they may have done it from just box 1, but it should be removed from all 3).
