The names in the header come from the first page of the form 1040, it doesn't indicate ownership of any kind just that it is part of your tax return so there is nothing to fix or change.
Look at all the other pages of the return ... everyone has your names & the primary's SS# ... this is a hold over from the days of paper returns... so that is a bunch of returns fell on the floor in processing then they could be reassembled properly.
