You don't receive the 8889, instead TurboTax generates it based on the information you supply in the HSA interview (Federal->Deductions & Credits->Medical->HSA, MSA Contributions) and attaches it to your return.
You say that you have an amount of $50 with code W in box 12 of your W-2? Is it possible that your employer accidently contributed $50 to your HSA before he/she realized that you didn't have one? (this happens sometimes).
If so, you need to talk to your employer and ask them if you have an HSA (in which you need to learn about it), and if you don't, will they issue a corrected W-2 so that you can do your return properly.
