Enter your W-2 exactly as is. Then complete a part-year resident WI tax return, and for your WI income enter only the portion of your income that you earned before you became a Florida resident. You became a FL resident for tax purposes on the day you moved into your new permanent home in Florida. WI will refund the excess withholding to you.
If you haven't done so already, advise your payroll department that you are no longer subject to WI withholding.
