Thanks for the quick reply and helping me find my mistake.  With additional research, I now understand that the HSA contributions are excluded from Gross Income and therefore were not included in my W-2 Box 1 Wages and Income.  So that explains why the $333 excess contribution gets included as "Other Income".  Form 8889 still seems confusing since Line 16 shows Taxable HSA distributions as zero, and the instructions say this gets copied into Schedule 1 Line 8f, which is 333 instead of the matching zero.  In the end, listing the $333 as other income achieves the task of getting it added to taxable income.
