If you select 'Revisit' at the beginning of the HSA section, select HSA for 'what type of plan?' You will then be asked if you received a 1099-SA. If you did, you'll enter the info, verify address, and are asked if you used it for medical expenses. If you didn't (or you already entered the info), the next screen should be a 1099-SA Summary. The very next screen will be the screen to enter your personal HSA contributions.
If you're not getting these screens and you've already cleared your cache/cookies, delete the Form 8889 from your return and step through the section again.
