If the income is related to a rollover, it is usually issued on a form 1099R. It's unclear why Fidelity would have issued you this specific form. You can try contacting them to request the appropriate form (1099R). 

 

If you can't get it corrected, or don't want to wait, you can enter the information on the form 1042s into TurboTax with this alternative:


 

  1. Identify how the income is coded on the form. This is usually shown as a code in Box 1 of the 1042S. It could be “interest, code 01” or “pension, code 15.”Form 1042S.jpg
  2.  Locate the section of TurboTax that corresponds to the income, for example “Retirement” or “Interest.”
  3.  Enter the withholding agent’s name and EIN from form 1042s (line 12a and 12d), and the gross income, into the appropriate field.
