Form 1095-C is used to provide information on whether you or any family members are enrolled in certain kinds of coverage offered by your employer (sometimes referred to as “self-insured coverage”).
You would not attach Form 1095-C to your tax return. You should keep it with your tax records.
Code 1E. indicates the following: Minimum essential coverage providing minimum value offered to employee and at least minimum essential coverage offered to dependent(s) and spouse. Do not use code 1E if the coverage for the spouse was offered conditionally. Instead, use code 1K.
