The Medicare premiums would be amounts reported on your SSA-1099 if you are collecting social security or that you pay out of pocket if you are eligible for Medicare but not collecting the social security.  You can also deduct any expenses for travel for medical treatment, like trips to the doctors office etc.  If you keep a log, the rate is 21 cents.  For New Jersey, however, you must have included the travel deduction on your Schedule A on the federal return.  @tbxuser
