My form from my provider (Payer) does not state, "TRUSTEE'S/PAYER’S name" as other do I guess. I had to figure that out on my own. The term "interview" is a term I'm unfamiliar with. For us non-tax experts it is a little confusing the first time around. Thanks for your answers and support!
